Insurance can be confusing. To help make it easier, these are the specific insurance packages from each insurance provider and whether Syzygy Health is in-network or out-of-network. This is not an exhaustive list, and it is your responsibility to confirm coverage with your insurance provider. If you have an HMO insurance plan you must call your primary care provider for a referral in order to be seen for gender-affirming hormone therapy or any GYN problem.
